Provides psychosocial assessments, crisis intervention, resources referrals, to facilitate discharge plans, and/or adjustment to illness: complex discharge planning for patients and their families; and resource referrals for families and friends of homicide victims and stabbing wound victims.
Works alongside the Transitional Navigators to facilitate discharge planning
Works alongside the medical team to facilitate adjustment to illness
Assesses stabbing victims' level of risk for repeated violence or re-injury and eligibility for a hospital based violence intervention program, will triage the situation, gather information to determine the risk of retaliation, provide practical assistance, and link to needed services at the time.
Connects stabbing victims eligible for the intervention program to intervention program personnel.
Searches for, identifies, and does outreach with formal and informal community victim support resources.
Coordinates appropriate referrals to community-based programs such as mental health resources, support groups, job-training resources.
Helps to coordinates post-hospitalization care alongside the Center for Family Services case manager and the Transitional Navigators for patient victims of gun violence or stabbing.
Follows up with patient victims of stabbings and families and friends of homicide victims routinely post-hospitalization.
Works with Center for Family Services as an outpatient to facilitate coordination of care.
Data collection for research as part of IRB-approved research in the needs of victims of violence
3 - 5 years health care experience preferred Education Requirements
Requires a Master Degree in Social Work ( MSW)/ or BSWor a licensed RN, BSN preferred Special Requirements
